Step 1: Read the Denial Carefully

A request may be denied because of the type of equipment requested, insufficient documentation, medical-necessity criteria, missing information, benefit limitations or another requirement of the specific plan.
Before doing anything else, families should obtain and carefully review the written denial.
Look for:
What was denied?
Why was it denied?
What policy or benefit provision was referenced?
Is there an appeal process?
What is the deadline to appeal?
Those answers help determine what should happen next.
Step 2: Determine Whether Additional Documentation Could Help
In some cases, the issue may not simply be the equipment.
The documentation supporting the request may need to explain more clearly why the mobility solution is needed for this particular individual.
Depending on the situation, supporting documentation may involve:
- Physician documentation or prescription
- PT or OT evaluation
- Letter of Medical Necessity
- Functional mobility information
- Positioning requirements
- Safety considerations
- Information about existing mobility equipment
- Explanation of the activities or environments creating the mobility need
The objective is not to generate more paperwork for the sake of paperwork.
It is to make sure the funding organization has the information needed to evaluate the request.
Step 3: Understand That Insurance May Be Only One Path
Funding options vary significantly by state, diagnosis, age, insurance coverage and individual circumstances.
Depending on eligibility and location, families may also be able to explore programs such as:
Medicaid
HCBS / Medicaid Waivers
California Regional Centers
State disability or developmental-disability programs
Foundations and charitable organizations
Community fundraising
or other individual funding resources.
Not every program will fund every piece of equipment.
But families should not automatically assume that an insurance denial means there are no other options.
Step 4: Build the Right Funding Strategy
This is where the process becomes individual.
One family may need to appeal an insurance decision.
Another may need to complete the insurance process before approaching another program.
Another may have access to a Medicaid waiver.
A California family may have a Regional Center pathway.
Another family may ultimately need to combine several resources.
